Tips For Referring Providers

Appointments are currently available in 1-2 weeks.

Neuropsychological Assessment

Your patient needs a comprehensive cognitive evaluation due to a known or suspected brain condition such as dementia or TBI.

Please include the following information in your referral:

  1. A detailed description of prior diagnostic evaluation (including neurological or mental status exam, lab or neuroimaging results, and screening measures) and why it was not able to clarify the diagnoses or questions to be answered by testing.

  2. That there has been a significant behavior, cognitive, or functional change resulting from illness or injury.

  3. The suspected differential diagnoses or other questions to be answered by the evaluation and how testing results will impact patient care.

Psychological Assessment

Your patient needs a detailed psychological assessment to help with differential diagnosis or determine effective treatment.

Please include the following information in your referral:

  1. A detailed description of prior diagnostic evaluation and why it was not able to clarify diagnoses or the questions to be answered by testing.

  2. Documented need for additional treatment recommendations after the patient has tried various treatments and remains symptomatic.

  3. The suspected differential diagnoses or other questions to be answered by the evaluation and how testing results will impact patient care.

Often not covered by insurance

ADHD Assessment

Your patient needs an evaluation for ADHD. Cognitive testing is not necessary for a diagnostic evaluation. If only a diagnostic evaluation is needed, please follow the guide under Psychological Assessment. If cognitive testing is also desired, please follow the guide under Neuropsychological Assessment.

Please note: Insurance rarely covers testing for ADHD if there are not significant complicating medical or mental health factors, such as severe mental illness, TBI, or seizures. If you refer a patient for an uncomplicated ADHD evaluation, please ensure the patient is aware this will likely NOT be covered by their insurance.

Pre-Surgical Assessment

Your patient needs a psychological evaluation for clearance for surgery for a bariatric surgery procedure or spinal cord stimulator.

Please include the following information in your referral:

  1. Type of surgical procedure.

  2. Any known mental health or substance diagnoses or relevant psychosocial factors that require evaluation.

  3. A statement that standardized testing is needed to evaluate preparedness for surgical intervention or psychosocial factors that may affect successful outcomes AND testing results are expected to impact the patient’s care and treatment.
